The Challenge
We are now tackling one of the biggest challenges facing the national health system: the growing impact of long-term conditions on people’s lives and on NHS capacity. Scotland faces a growing tide of long-term and multiple health conditions—especially among an ageing population—which significantly burden both individuals and the health system.
The Solution
Empowering citizens to manage their care is crucial and urgent. If they are enabled, individuals can become confident partners in managing their own conditions, supported by appropriate information, services, and relationships. Working closely with national partners, councils and charities, we are developing best-in-class digital tools for supported self-management. Our partnerships include projects developing digital pathways for weight management, falls prevention, colorectal cancer and people living with MS.
The Impact
Our ambition is to develop digital solutions for enabling and scaling system change for better health outcomes:
The Trauma App – With 30+ hospitals using or trialling and 2500+ trauma cases documented, the result is a tool that empowers clinical teams during high-pressure interventions, improves patient outcomes, and significantly reduces administrative burden.
ScribePro – trusted and used by major organisations including Scottish Rugby, the Belgian National Association, and the National Football Association of Scotland. It is also in live test use by FIFA, UEFA, and national football associations including Egypt and Ireland.
The supported self-management tools are still in prototype and pilot phases and are already demonstrating value in enabling citizens to feel more able to manage symptoms and treatments. This is the foundation for achieving the ambitious aims of our partners to deliver these tools at scale across Scotland.
Future Plans
We are working with the Scottish Government, NHS Scotland, the National Digital Platform and third sector partners to design and deliver digital self-management tools.
These digital tools will support any health pathway, empowering those with long-term conditions to manage their health with confidence. Our vision is for self-management to become the norm across Scotland, reducing demand on services and improving health outcomes.
