Executive Summary 

Infix Support (Infix) was founded by consultant anaesthetist Dr Matthew Freer in 2019, at which time Matthew was working at NHS Forth Valley. 

The company’s main product, Infix Schedule, is optimised system-wide scheduling software that increases planning efficiency and theatre productivity, integrating with existing healthcare legacy systems. 

In May 2024, Infix won a NHS Scotland contract aimed at reducing hospital waiting times, optimising operating theatre efficiency and accelerating the patient pathway nationwide, following a public procurement process.  

Infix Planner, which is a tool to plan activity across multiple hospitals, has been developed and purchased by SpaMedica. They are the largest provider of private ophthalmology procedures in Egland, with over 60 hospitals.  

Infix recently commenced a trial of their Infix Tracker solution with PureHealth, the Middle East’s largest hospital operator, at Sheikh Khalifa Medical City in Abu Dhabi. The solution will track patients readiness for surgical treatment and automate workflows, resulting in less cancellations which will lead to increased revenues. 

The Challenge 

The primary challenge faced can be characterised as the significant inefficiencies within operating theatres, whereby surgeons and nursing staff were frustrated by the poor planning of theatres and the lack of incentives to improve productivity.  

Historically, there had been a reliance on manual processes, paper and human estimates of operating times, while not accessing the mass of NHS data to help create more positive solutions.  So, how could we improve the workflow in theatres to enable more operations to be completed?  

The Solution 

Delivering smarter, more connected and efficient workflows using our proven expertise, technology, AI capability, and cloud-based tools. The flagship solution, infix schedule, is backed by over 3.4 million NHS operating times, which delivers a significant improvement in predicted optimisation of surgical operation lists. Infix has increased their product range to 5 cloud based solutions, all of which aim at improving OR efficiency and productivity. 

The Impact 

Following on from trials with NHS Health Boards which demonstrated that Infix’s technology increased operating theatre efficiency by up to 30 per cent, in 2024 after a public procurement process Infix was awarded a NHS Scotland contract aimed at reducing hospital waiting times, optimising operating theatre efficiency and accelerating the patient pathway nationwide.  

By August 2025, Infix has been implemented at NHS Lothian, NHS Forth Valley, NHS Highland, NHS Lanarkshire, and NHS Greater Glasgow and Clyde, Scotland’s largest NHS health board, with the implementation at the remaining health boards set to be rolled out over the next few months. The solution has continued to improve theatre efficiency by up to 20%, and with over 2 years worth of post implementation data have increased the prediction of surgical lists from 8% to 91%. 

The success of Infix in Scotland has led to commercial opportunities and engagement across the UK and internationally, including the recently commenced pilot with PureHealth, the Middle East’s largest hospital operator, at Sheikh Khalifa Medical City in Abu Dhabi. 

Future Plans 

Continued rollout across NHS Scotland health boards, with all hospitals in Scotland online by Q2 next year. 

NHS England – some tentative connections made as there is a stronger push for increased productivity at reduced costs. All of which we have data to prove we can deliver.

Our International strategy at present is to explore expansion in the Middle East, concentrating on UAE and Saudi Arabia. We are on the AHBI, Saudi accelerator so have started making some strong connections in healthcare providers. This area is going through a rapid digital transformation in healthcare, and with our trial in Pure Health it is giving us the credibility to approach other clients. 

Product development – We are further enhancing our AI technology within our products, and specifically with infix schedule we have the ability to build in probabilistic scoring to further improve theatre optimisation. This uses AI to analyse surgical waiting lists and instantly score patients that can be added to operation lists with a reduced risk of over-running the theatre. Also our AI validation tool has allowed us to continually monitor the prediction accuracy of the scheduling tool and offer clients comfort that the software will adapt to the changing license while maintaining accuracy levels of >90%. 

We are in a good financial position but the chance to accelerate our international growth has allowed us to explore various UK and international investment opportunities.
